FAX Information Line

The index listing all documents available can be obtained by FAX by requesting document number 0000.

Each of the individual document numbers is listed on the table of contents for a given document type (e.g. Advisory Statements, Position Statements, Opinion on Ethics). The document number also appears at the bottom of each individual document.

To have any of the documents sent through your FAX machine:

1. Dial 1-800-999-2939 for the United States, Canada, and much of the Caribbean.
   Dial 1-847-391-0908 for International access. You will be prompted for country code, city code, and phone number. Do not enter "011"; the system will add this prefix automatically.
   
2. Select option 2 at the voice prompt.
   
3. Enter the corresponding four digit number for each document you wish to receive.
